Dear all members, to get benefits for the elegible members it is required to Re Submit your application form to the Samaj Office as soon as possible. The last date for the Request form id June 30, 2020.
Find the detailed information about the elegible criteria :
- For the Benefits for Medical and Education, the total Family income should be less then or equal to 10,000 per month. This needs to be verified by Form 16.
- The Bank statement of all family members for the 1st April, 2020- 31 May, 2021
- For the widowed benefits, Husband's death certificate is mendotary.
- The education and medical bill older then 2 month is not be acceptable and can not be submited.
- The copy of Electricity bills of last 1 year needs to be attached.
- Salary certificate will be required from the employer on letter head with Stamp, INcluding your PAN number. The address of Office, Contact Details and information about the owner is also needs to be mentioned on the application form.
- Your Bank account is mendotary, all the cash benefts will be transferred to you through NEFT.
- Your application needs to be attested by the ZONE Head.
- Computerized saalry slips are not acceptable.
- Samaj needs up to 2 months to approve your application.
- All rights reserved about the help done by Mahesh Seva kosh. Your financial status can be examined any time by any samaj Officials and if found dissatisfaction, samaj has rights to terminate your benefits.
